Commerce Public Schools has arranged for sports physicals to be offered at the Commerce High School gym from 9am-3pm on Wednesday, June 3rd. These physicals are for all students entering grades 6-12 that wish to participate in athletics. Social distancing will be practiced.
The cost of the physicals will be only $10. Please bring a check or exact change. The OSSAA physical examination form will be available at the high school workouts starting Monday morning or you can CLICK HERE to print your own. The page titled OSSAA PHYSICAL EXAMINATION AND PARENTAL CONSENT FORM needs to be completed in it's entirety by a parent before seeing a Dr on Wednesday for a physical.
This will be the only day the school is able to offer physicals and if you're unable to attend that day it will be up to the student and parents to get a physical before the student can participate in the fall. The physicals outside of this date could be considerably higher priced. Please be mindful of the healthcare workers' time and do not expect to show up at closing time and receive a physical.
